Conditions of Grooming

Samford Pet Resort (SPR)
4 Paws Grooming and Spa

1. MR/Mrs......................guarantees that ..................... has up to date vaccinations covering his/her entire stay and has given clear and complete grooming instructions, and has understood the pricing for grooming services requested before those services are performed by Samford Pet Resort (SPR.)
2. MR/Mrs........................agrees to pay all costs and charges not later than the day of pick-up. All charges are non refundable.
3. MR/Mrs..............................agrees to pay all veterinary services and costs for...............for any veterinarian treatment that may be needed in the case of an incident or serious medical problem being discovered during grooming. MR/Mrs...................... also agrees that SPR, its owners, agents and operators are not liable for any injuries, clipper burns and problems found during grooming.
4. SPR shall exercise reasonable care for the pet. MR/Mrs............................... accepts and recognizes that while at SPR for grooming services, the pet may possibly contract an illness, and will not hold SPR, agents or operators responsible for any expenses or charges incurred by MR/Mrs............    for Veterinary services or medicine after the pet’s stay.
5. SPR retains the right to decline acceptance of a pet if found to have a medical condition or exhibits behaviour that renders the grooming service too dangerous to perform for the safety of the groomer  and the dog.
6. If the groomer finds a tick whilst grooming a full clip will be provided to ensure the welfare of the pet. MR/Mrs..................agrees to pay all costs for the grooming of the dog including but limited to special handling, and tick control.
7. MR/Mrs............agrees to be solely responsible for the behaviour of ....................while he is in the care of SPR.
8. While at SPR we will exercise all necessary precautions during the grooming process, MR/Mrs..................... is aware that if ..................... has a badly matted coat, it may be necessary to shave or de-matt ............. and that these procedures may have unpleasant consequences such as clipper burn, brush burn, or nicks and cuts, skin redness, itchiness or self inflected irritations/abrasions, SPR will use all precautions during these procedures but will not be held responsible for the aforementioned side effects.
9. MR/Mrs.....................agrees that if..................... is not collected by closing time then SPR Boarding rates and contract will come into effect and will need to be paid when...........................is collected the following day.
10. Owner agrees and accepts for SPR’s right to take and use pictures of the said pet in our care whether for own records or marketing use.
11. A soft muzzle may be used, or services discontinued for the well being of your pet, should the groomer or pet be at risk of injury.
12. Owner acknowledges responsibility to notify prior to grooming, if the dog has bitten or has aggressive tendencies.
13. Owner understands they will be held liable for all bites caused by the dog.
